Submitting Application for Permanent Care

If you wish to submit an application for placement to Donwood we required the following information:

An Aged Care Client Record (ACCR)
To enter Aged Care all applicants are required to have a valid Aged Care Client Record (ACCR).
The Aged Care Assessment team usually visit your place of residence and do an assessment on all your care needs. You will receive a copy of the ACCR once the paperwork is completed by the Assessment Team. Please include this assessment with your application.

The ACCR is free (funded by the government)

An assessment from Centrelink/DVA for means tested care fees
The Request for Combined Income and Assets Assessment form is included in the information pack provided and needs to be completed and forwarded to Centrelink/DVA.

You will then receive advice from Centrelink/DVA about your Means Tested care fee. Please forward a copy of the advice you receive to us.

Power of Attorney – Financial and medical
If you hold an Enduring Power of Attorney and/or an Enduring Medical Power of Attorney please include them with your application.
 

What are the costs?

There are three types of payments for residential aged care:
•  Daily Care fee
•  Means Tested fee (for some residents)
•  Accommodation payments (Entry Fees)

Daily services fee
This covers daily living costs such as personal and nursing care, meals, heating and laundry. They are paid by all residents, and are determined by the Department of Human Services.

Means Tested Fee
To assess your eligibility for means tested care fees you are required to complete an Asset and Income Tested form and submit to Centrelink/DVA depending on the type of pension you receive.

Centrelink/DVA will advise you via a letter once the assessment is completed on what means tested fee you will be required to pay, if any.

Depending on individual circumstances we may choose to charge an interim means tested fee until we have received advice from the Department of Human Services, Department of Veteran Affairs.

Donwood in no way benefits from this fee. The government reduces the subsidies we receive for the care we provide to the resident.

To assist you with calculating what fees may be payable you can log on to My Aged Care estimator. You will need to have all the information regarding your financial affairs to answer all the questions to get the most accurate answer.

Means Tested care fees are capped.

Each year you can only be asked to pay a, maximum of $26,380.51. This amount is indexed each year.
Once you have reached this amount you will not be asked to pay means tested care fees until your admission anniversary.

There is also a lifetime cap.
You can only be asked to pay in your lifetime $63,313.28.
This cap in indexed.
Once you have reached your cap you will only be asked to pay the basic daily care fee and your agreed accommodation costs.

Accommodation payments (Entry Fees)
These payments contribute towards the cost of accommodation in a residential aged care facility. It is only paid if the resident's assets exceed an amount determined by the government.

The payment method must be decided within 28 days of entering care.

The government stipulates all residents must retain a minimum asset value which is currently $47,500.

The entry fee (lump sum component) is fully refundable when the resident departs. A certificate of probate is required if the resident passes away. Upon receipt of probate certificate the entry fee will be refunded in full within 14 days.
Payment method can be either:
•  Refundable Accommodation Deposit (RAD); or
•  Daily Accommodation Payment (DAP) ; or
•  a combination of both.

Donwood offers two levels of accommodation for rooms in its facility:

 

Single Rooms – Diana Street Lodge/Donwood Nursing Home

Single rooms in the Nursing Home, Banksia and Callistemon, Acacia (memory support) are priced at a Refundable Accommodation Deposit (RAD) of $450,000.

These rooms can also be secured by the payment of a Daily Accommodation Payment (DAP) of $71.26 per day.

New residents are also able to pay by a combination of Refundable Accommodation Payment (RAD) and Daily Accommodation Payment (DAP).

An example of such a combination payment is as follows:
Part payment of RAD (Lump sum component)                   e.g $200,000
Part payment of DAP                                                          e.g $200,000
Equal to                               $200,000 x 5.78%
                                                  365 days                           = $39.59 per day


Therefore, in this example, a room priced at $450,000 might be secured by a payment of a RAD of $200,000 (lump Sum) and a DAP of $39.59 per day.

The lump sum is fully refundable upon discharge.


Shared Rooms – Nursing Home (only)

Shared rooms with two occupants are priced at a Refundable Accommodation Deposit (RAD) of $350,000.

These rooms can also be secured by the payment of a Daily Accommodation Payment (DAP) of $55.42 per day.

New residents are also able to pay by a combination of Refundable Accommodation Payment (RAD) and Daily Accommodation Payment (DAP).

An example of such a combination payment is as follows:
Part payment of RAD (Lump sum component)                     e.g $200,000
Part payment of DAP                                                            e.g $150,000
Equal to                                       $150,000 x 5.78%
                                                           365 days                    = $23.75 per day


Therefore , in this example, a room priced at $350,000 might be secured by a payment of a RAD of $200,000 (lump sum) and a DAP of $23.75 per day.

The lump sum is fully refundable upon discharge.

Fully Supported Residents
Residents assessed as having assets less than $47,500 and income below $26,072.80 (single person) may be approved by the Government as “fully supported” residents and will have their accommodation costs fully paid by the government.

Daily care fees remain the same. Increase in daily care fees occur twice a year in March and September when the pension is reviewed.

Supported Residents

Some rooms are available for residents with reduced financial means.
Supported residents may be asked to contribute to their cost of care via a Daily Accommodation Contribution (DAC) or Refundable Accommodation Contribution (RAC), or a combination of both. If a persons assets are between $47,500 and $162,087.20 and their income is over $26,072.80 (including their age pension) they may fall into this category.

 

 

All examples are a guide estimate and dollar figures may be changed depending on circumstances.
